Here’s what to expect when reaching out to Rize:
A Rize educator will reach out to learn about your child’s challenges and needs.
We’ll gather important information so that we can better guide you and help make sure that you find the match and support that’s right for your family.
You will be contacted to discuss the best fit for your child — location, language, gender, preferences etc. A devoted team will guide you through all the necessary forms and paperwork.
A licensed professional, such as a social worker or care manager, will meet with you and your child, either at home, school, or virtually, to get a complete picture of your child’s strengths, needs, and goals.
Based on the assessment, a custom plan will be developed outlining the services, support, and goals designed to help your child grow and thrive.
This plan is shaped with your input and reviewed with you before moving forward.
Once everything is approved and your provider is cleared, your child will begin receiving services — at home, school, or in the community.
Your care manager or coordinator will continuously remain in touch to monitor progress and adjust as needed.
